This instrument is designed to diagnose Autism Spectrum Disorder (ASD) in children. A child is considered high-risk for ASD if they answer “Yes” to any 4 negatively worded items or “No” to the corresponding positively worded items out of the 21 total items. The instrument consists of 21 items with dichotomous (Yes/No) responses and is intended to be administered to parents or caregivers.

Instruction: Indicate whether or not you have observed any of the following behaviors in your child.
