Social Anxiety Scale

This instrument measures the intensity of anxiety/fear and the degree of avoidance in social and performance situations. It contains 23 items and is divided into two subscales: performance situations and social interaction situations. Items are rated 0–3. For fear/anxiety, the scale represents: 0 = None, 1 = Mild, 2 = Moderate, 3 = Severe. For avoidance, the scale represents: 0 = Never, 1 = Occasionally, 2 = Often, 3 = Usually.
